in regards to my shop lease which has 7 years left @ 12k per annum, do i enter this on the form against my landlords name i.e MR J Smith - £86k?

Q3? Where? Section 3 is about assets so I'm a little confused (I shouldn't worry that's not difficult to achieve).
Certainly you should enter the outstanding lease details as a sole trader in section 4 unsecured.
