Also can I include water rates as they have been passed to a debt recovery service and will this affect my services when declared bankrupt.
Many Thanks

As Richard P says, the water rates debt should be included. The water company can't refuse to supply you because you are bankrupt, and shouldn't add the arrears pre bankruptcy to a post bankruptcy bill (although they may try to) - however I have seen cases where they request a deposit to secure future bills, although I'm not sure how widespread that is.
As far as listing your original creditors and the agencies instructed - I'd also put both down - it'll reduce the chasing letters you get post bankruptcy
Best wishes

I listed both the original debt and the vermin that purchased the debt (oops sorry old habits die hard)
By doing so it meant that the OR sent letters to every one on my behalf. yes it made my BR figure a lot higher but that did not have an effect on me
on the form I entered in the right hand column, original debt and cause or duplicate entry debt obtained from company XYZ
The water rates can be listed, the water company cannot cut off your supply. I am not certain if the water company can make arrangements with you to get the arrears back at a later time as they have to supply the water to you.
If they did this it would be preferential treatment to a creditor but it means you have water
