Posted - 12 September 2008 : 21:20:06 just a few questions (i promise) section 6.9, whats my tax reference number? how do i find out the address of my tax office? and finally the credit card section, whats it mean by the date the debt was incurred?

As Jo says, your tax reference number (which is not your tax code) and the address of your tax office is detailed on your P60 which you receive annually.
The date the debt was incurred was the date you first obtained the credit card.

The date incured i put as a rough guess that date in which you took out the card. I was not 100% sure on the dates but wrote something in.
We found tax office details on the P60 that we had from our employers and also found the tax ref number too on the same form.

I phoned my HR department to ask them my tax reference number and the address of my tax office.
Hope that helps. Those forms are a nightmare aren't they?! Hope you've nearly finished now!
