re 6.28. Yes you put your name, address and occupation. The sworn at section is to be completed at the court on the day together with the bankruptcy clerk.
You need put nothing where it says " several pages marked"

Re: the affidavit, do I fill in my name, occupation, address in this section? the bit about make oath and say that the several pages marked...................and contained in the exhibit marked "Z". What does this all mean. and what do I put in next to the "sworn at" section. Help!!

no not normally. Each part of the forms is relevant to you and to be completed by you. However most clerks will give a little assistance when you arrive. The danger is if they are not very obliging you could be turned away if the forms are not completed properly.
Which part of the 6.27 are you finding difficult? There is quite a bit of repetition I know.
