Posted - 12 April 2009 : 08:48:07 When filling on online forms 1) where it says any other names do you put your maiden name and 2)where it asks if you have paid any payments to creditors do I mention iva, thanks I'm sure I'll have to ask loads more questions as the days go on.

1) Yes you should enter maiden name
2) You do not need to enter IVA payments as both the 6.28 and 6.27 forms identify the IVA.
