if you declared bankruptcy yourself the Statement of Affairs clearly asks if you have ever been known by any other name. This includes a maiden name in the case of a married female.
If you were declared bankrupt by a creditor the same question is asked on the IPOQ questionnaire. I assume you have completed one or other of these forms and if you have not disclosed your previous name then you could, if found out, be accused of perjury.
