Hi

if you declared bankruptcy yourself the Statement of Affairs clearly asks if you have ever been known by any other name. This includes a maiden name in the case of a married female.

If you were declared bankrupt by a creditor the same question is asked on the IPOQ questionnaire.
I assume you have completed one or other of these forms and if you have not disclosed your previous name then you could, if found out, be accused of perjury.

Not sure if it is a legal requirement, but it's probably best to be upfront anyway.
