Do I have to take any proof of income (ie benefits awards as I am unemployed and on benefits, housing benefits, rent agreements, bank statements etc) or do I just submit the completed application form with the fee

You only need to take 3 copies of the SOA and the £510 fee or 1 copy of the SOA and £522 fee if you are petitioning at the RCOJ.

If you are on benefits you can claim some of the fee back (£160 I think) by completing and submitting the EX160A and have a letter stating you are receiving the benefit. This needs to be submitted within 1 month of petitioning for BR. dated within 1 month of tomorrows date.
