On form 6.28, at the top of page one, it says:

Insolvency Act 1986

In the

I Bankruptcy No____________ of 20_____

Re_________________________________________________________

Then in the guidance notes it states: 'Details will be the same as those shown at the top of your petition'.

What is all this referring to please? At the top of the petition it just requires my name and address!

That section will either be filled in or stamped at court when you actually hand in your petition.
