I missed out that I would like to pay £20-£40 pm to my childrens Trust Fund. I used to pay this but stopped whilst on maternity. I would like to set it up when I go back to work next week.
Should I add on section 13 that I forgot to add this and ask if it allowed?

15 hours to go!

Me again, yes add it on an extra piece of paper like you say in section 13.
