How do I complete this form for the OR? I dont understand it?

It says to number - how do you number them? Then Date of first and last entry - what is this?

Hi

goes something like this

1. Blue file, details with :- a) lloyds tsb bank records 2001 - 2008
b) capital 1 credit card 2004 - 2008


then

2 red folder - contents :- etc etc etc
