where on the form do i put a ccj debt just in unsecured creditors

If unsure, You could put it in unsecured creditors and then put a note at the side of it saying see section 13 (Extra information) and put an explanation of the debt in Section 13.
As long as it is on there, at least the OR has the information

list with all other unsecured creditors but put (CCJ) after name of creditor, and provide references etc in 'additional information'
