Just filling it out what do you put in the place for date incurred on creditors section not really sure on credit cards etc because the debt was not incurred on any particular date but over a period of time ?

'Number of years' can be placed here, in essence this refers to loan dates and not credit cards as in when were the loans taken out, so don't worry
