Cheer up its a lot to take in, and seeing as you have had to wait a long time for your interview things are really racing through your head. Honestly trust me i know im no Dr but it will be OK!!!!

Put what you think is about right, just mention it when you have your interview and add on section 13 an extra bit to say that the dates are approx.
You will be fine. Try not too worry. :-)

No we dont all keep a mental notes of the exact dates. Some things like loan agreements will show others wont. I would not worry.

I would not worry. They can find out if they need too its just so that they have a rough idea, i could not remember all mine so i just put 1/1/05.
Sounded a good date and i just told the OR that i could not remember. :)

If it's in the last 2 years enter the approx month & year.
If before that the year should be fine. If you can't remember the older dates give an estimated date and the word "approximate."
