The form you need (with instructions) is below:

http://www.hmcourts-service.gov.uk/courtfinder/forms/ex160a_e.pdf

http://www.hmcourts-service.gov.uk/courtfinder/forms/ex160.pdf

It also lists the benefits you need to be claiming in order to be able to claim the £150 fee reduction.

Hi malcolm

if you are refering to the £150 you collect the form from the court

different procedure different courts, some you complete the form on the day and you get the reduction. others are more slow minded and you get the credit returned to you a few days later

the best suggestion is ring or visit your local court ask to speak to the Bankruptcy clerk

good luck Richard
