| Hi,do I fill in first page of bankruptcy form and what about affidavit,will it be sworn in court,so I fill in name occupation and address or wait |

First page of 6.27? Name, address, occupation, previous names and addresses, any business details. And sign it in court.

Hi,it is first page of 6.28.it has got affidavit,says when u have filled in forms affidavit. must. Be sworn before. A solicitor or commissioner for oaths or an officer of the court duly authorised to administer oaths

The court officials are authorised and, as Viki says, you sign it when you are at court.
