I know I've asked a very similar question before and I apologise for repeating myself but where on the online forms is the space for additional information?

I believe from memory when you print it out it comes out as a blank (but numbered etc.) page for you to fill in by hand. I think that's right but forgive me if someone corrects me.

I think that's right - I seem to remember reading that in the last couple of days.

All i did was photocopy the page once i printed it and wrote it in once by hand then photocopied that to hand in.
